1. By using Pinnacle Performance Medicine Men’s Health Clinic, you agree to these Terms and Conditions.
2. Scope of Services: Care is delivered by licensed
providers via telehealth. We offer medical consultations, ongoing management, and prescription services as medically appropriate. We do not provide emergency or in-person care.
3. Patient Responsibilities: You must provide accurate health information, disclose relevant medical history, and keep your contact and payment information current.
4. Telehealth Consent: By participating, you acknowledge and consent to the use of telemedicine and understand its limitations compared to in-person care.
5. Financial Policy: All consultation, program, and medication fees are due in advance. Full details on fees, refund, and cancellation policies are available on our website and intake forms. Medications, once prescribed and shipped, are non-refundable.
6. Privacy: Your information is protected as required by HIPAA. We use secure platforms for all communication.
7. Program Renewal: Monthly programs renew automatically. You may cancel with the required notice period before the next renewal to avoid future charges.
8. No Emergency Services: If you have a health emergency, call 911. We do not offer urgent or emergent care.
9. Liability: While we provide care to the best of our ability, we do not guarantee results and disclaim liability except as required by law.
10. Updates: Terms may change without notice; continued use after updates constitutes acceptance.
